“Pay Small Small” — financing UHC for the informal sector.

A field note on the flexible-payment scheme designed to bring informal-sector residents into Lagos’s coverage system without breaking them.

A street trader in Lagos pushing a cart.

Lagos’s informal sector represents the majority of the state’s working population. Bringing those residents into the formal health insurance system has been the most stubborn problem in state-level universal health coverage. "Pay Small Small" is a flexible-payment product designed to meet informal-sector workers where they are.

This field note is being written. It is intended as a quick, operational record of the product — the design choices behind it, the early data on uptake, and what the experience suggests for similar products elsewhere on the continent.

What the field note will cover

  • · The informal-sector enrolment problem.
  • · The design of the “Pay Small Small” product.
  • · Early operational data — what enrolment looks like in practice.
  • · What is and isn’t translating.
